Petition/Notice of Defense on the form set forth in the California Code of Regulations (CCR) Article 12, Section 3911, or provide 1) written statements, with supporting documentation, indicating specifically, the basis for the petitioner's challenge of the DMR's order of administrative penalty, and 2) a written statement advising the Board of the name, address and telephone number of the petitioner's representative, as set forth in CCR Section 3901.
